Safeguarding Concerns

Use these forms to report new safeguarding concerns for a vulnerable adult who may be experiencing, or is at risk of abuse or neglect, or to provide additional information about an existing safeguarding concern.   When submitting a concern, provide as much information as possible about the adult, the nature of the concern, any immediate risks, and details of those involved. If you are updating an existing report, please include the reference number of the original concern if possible. If somebody is in immediate danger, a crime is in progress, or urgent medical assistance is required, call 999 rather than using this online service.
