Use this service if you are a Homecare Provider and need to request support relating to a service user, or to report a No Entry where a scheduled visit could not be completed. Requests may relate to concerns about a person's wellbeing, changes in circumstances, support requirements, or difficulties affecting the delivery of care.   Please provide as much relevant information as possible, including details of the service user, the reason for the request or No Entry, actions already taken, and any identified risks. For No Entry reports, include the date and time of the visit, attempts made to gain access, and any welfare concerns.   If there is an immediate risk to the person's safety or welfare, contact the emergency services on 999 rather than using this online service.
