Care and Support Requests

Use this service to request Adult Social Care support for yourself or someone else, or to provide additional information about an existing Care and Support request. Requests and updates can be submitted by members of the public, family members, carers, advocates and professionals.   Before submitting a request, please have as much relevant information as possible, such as the person's contact details, circumstances, support needs and any significant risks or recent changes. If you are providing an update, please include your reference number where available. In an emergency or where there is an immediate risk to someone's safety, call 999 rather than using this online service.
